The Marigold Survivorship Medical Clinic

The Marigold Survivorship Medical Clinic is a specialty telemedicine clinic for women affected by breast cancer. A clinic by a young survivor for other survivors.

Founded by Dr. Caroline Cook, the clinic focuses on menopause management, lifestyle medicine, sexual health, recurrence risk reduction, and the long-term health effects of breast cancer treatment and early menopause.

Through evidence-based care and personalized guidance, my goal is to help you improve your quality of life now while protecting your long-term health.

Who is Dr. Cook?

Dr. Caroline Cook, DO, is a triple-positive breast cancer survivor who was diagnosed at age 37. Her personal experience with an unexpected breast cancer diagnosis, navigating chemotherapy and recoveries, seeking surgical options, and abrupt premature menopause with all the associated concerns shaped the vision for The Marigold Clinic.

She is a Menopause Society certified practitioner, board certified in lifestyle medicine, and a practicing board certified emergency physician. Through both her medical training and personal experience, she recognized that many young women with breast cancer are left navigating menopause symptoms, long-term health risks, and lifestyle changes without adequate guidance or support. Throughout her own care she did not know where to find the care she wanted, so she decided to create it herself.

Caroline strongly believes that lifestyle medicine and traditional medical therapies work best together — and that when both are used thoughtfully, women can feel dramatically better and improve their long-term health and quality of life.

Most simply, her goal is to help women live well after breast cancer.

Outside of medicine, she enjoys cooking, reading, hiking, biking, skiing, yoga, traveling, coffee, time with friends and her husband, and sleep. She also helps lead a young adult breast cancer social group in her local community.

Schedule a time with us

We will begin with a complimentary 15-minute phone or video call to discuss your needs, review the services offered at The Marigold Survivorship Medical Clinic, and answer general questions. This call is an opportunity for both of us to determine whether this would be a good therapeutic fit. No medical advice will be provided during this visit.

If you choose to move forward, you will schedule an intake appointment. Prior to this visit, you will be asked to provide a checklist of medical records related to your oncology care and complete a detailed intake form outlining your medical history and goals. Please allow approximately one week for record review prior to your intake appointment.

Initial Intake Visit: $400
Your intake visit will require at least one hour, and sometimes up to 90 minutes. During this visit, we will review your history in detail and begin developing a personalized plan to address your specific concerns and goals.

Follow-Up Visits $300
Follow-up visits are typically 45–60 minutes and are used to monitor progress, adjust treatment plans, review labs, and continue refining your care.

Weight Management and GLP-1 Medications: $200
If you are pursuing weight management with GLP-1 medications, visits will initially be scheduled every 4-6 weeks. These visits will include discussion of nutrition, physical activity, side effects, laboratory monitoring, body composition, and rate of weight loss to ensure a safe, evidence-based approach that optimizes your success with treatment.

What services do you offer?

I offer telemedine visits in menopause (including HRT), sexual health, and lifestyle medicine for breast cancer survivors. I also offer coaching for weight loss and GLP1 medications. Currently seeing patients in Oregon, Washington, and California. I am also happy to see women without breast cancer on a case by case basis.

If you are looking for a hormone therapy, please be aware at this time I am unable to prescribe testosterone in the states of California and Washington due to DEA regulations .


What is Lifestyle Medicine

Lifestyle medicine is an evidence based field of medicine that focusses on preventing and treating disease using the six pillars of lifestyle medicine. These include: physical activity, food and nutrition (whole food plant predominant), restorative sleep, stress management, social connections, and avoidance of risky substances. Dr. Cook strives to enhance the lifestyle of all her patients to improve their quality of life. However, she does also believe pharmaceuticals play a role and are not a failure.


Do you take insurance

We are out of network with all insurance currently. However, you will receive a superbill with codes that you can submit to your insurance for reimbursement depending on your policy. You can use HSA/FSA for all care at The Marigold Survivorship Medical Clinic.
